Restore Forms dialog
This dialog allows you to restore forms that have been deleted.
The dialog is displayed when the Restore... button of the Manage Forms dialog is clicked.

Lists details of forms that have been deleted and can be restored. |
| History | Opens the Form History dialog for the form selected in the list. |
| OK | Restores the form selected in the list. |
| Cancel | Closes the dialog without restoring any form. |
